Guidelines for completion of Employee of the Year Nomination form:

  1. Nominations are open to all staff who are full time, part time or per diem employees.
  2. Nominations need to be written in the third person (no names) by professional peers
  3. Collaboration among the staff is encouraged
  4. Deadline for submissions is May 2nd
  5. Late submissions will not be considered

This award recognizes an outstanding employee who embodies all aspects of our standards of performance and values.  This employee upholds our mission and vision each and every day.  In addition, this individual puts quality patient care at the top of their priority list, while working to maintain a positive culture at Pascack Valley Medical Center.    

List the nominee’s title and department without mentioning the person’s name
How does this person live up to the standards of performance? How does this individual uphold the company values? How does this individual contribute to the culture at the hospital? What positive impact do they have on patient care?
What has this person contributed perhaps to the community or profession that is noteworthy? Where/how has he/she gone “above and beyond” What professional or charitable organizations is he/she a leader of?
Discuss any human interest aspects to his/her life that is different or special, or unique that you want the judges to know
